This survey study reports on the “real-world” management of testicular torsion with
respect to the recommendations published in the EAU Paediatric Urology guidelines

]. The greatest number of respondents were from The Netherlands (71%) and thus, most
of the results reflect Dutch practice patterns. Or, more accurately, it reflects the
practice patterns that are immediately recalled upon filling out the survey, since
82% report treating fewer than 5 cases in the past year. Although the authors found
that most responders adhere to EAU guidelines, a sizable proportion (23%) do not pursue
emergency surgery, nor do they perform fixation of the testis, despite the published

- PreviewWe are very grateful for the supportive commentary done by the authors on our paper [1]. Clearly the commentary has raised some important aspects of European Association of Urology (EAU) guidelines that were not observed by 23% of the respondents. Perhaps they were not aware of the guidelines but more worryingly the medicolegal and patient-related consequences of a further episode of testicular torsion or missed torsion leading to testicular loss.
